• We do NOT collect personal information via text messages.

• Insurance card information
• State issued identification cards such as driver’s license.

• Primary care provider information

We use the personal information we collect to communicate with you to provide timely medical care such as:
• Appointment set up and reminders
• Updates on treatment plans and billing.
With Whom This Information Is Shared
We do not sell or rent your personal information. We may, however, share your personal information in limited circumstances, such as with the 3rd party billing company, SHuR Can Services, your own insurance company and other specialty clinics when referrals are necessary for further advanced care and to other parties that you designated to share your information such as your attorney and primary care providers. At times with your permission, we may share with coordinators for research opportunities related to your medical conditions and other medical device companies. We require our service providers to keep your personal information secure, and do not allow them to use or share your personal information for any purpose other than providing services on our behalf.
What Choices You Can Make About Your Information
If you wish to unsubscribe from text messages, you can call our office and express your wish to any of our staff that you will no longer receive text messages. Please note that unsubscribing will not remove your information from our medical record
system.
